Supplementary Health Insurance in Italy: Why Private Plans Still Matter

Italy has a public health system. Residents can use it, and employees contribute to it through payroll. So a US founder often asks a fair question: why add private health insurance on top?

The short answer is that the public system and a private plan do different jobs. Most Italian employees use both. If you want to hire and keep good people in Italy, you should understand the gap between them.

What the public system does well

The national health service, the Servizio Sanitario Nazionale, covers a broad range of care. Family doctors, hospital treatment, emergency care and many specialist services are available to people who are registered. For serious illness, the public system is a core part of the safety net, and many Italians trust it.

You do not need to replace it. A private plan is not a substitute for it. It sits next to it.

Where the friction shows up

The pain points are practical, not medical.

Waiting times. For non-urgent specialist visits and diagnostic tests, waits can be long. How long depends on the region, the local health authority and the type of exam. An employee with a nagging problem may wait weeks or months for an appointment.

Regional differences. Health care is organized at the regional level. Quality, speed and access are not the same everywhere in the country. A team member in one city may have a very different experience from a colleague in another.

Choice of doctor and facility. In the public system, you often go where you are assigned or where there is space. Private plans let people pick a specialist, a clinic and a time that fits their working day.

Time away from work. Long waits and slow scheduling can pull people out of work for longer than needed. A faster route to a diagnosis helps the employee and the business.

Language. Foreign hires and relocated staff may be more comfortable with providers who speak English. Private networks often make that easier to find.

What supplementary plans typically cover

Plans vary by insurer and by contract, so read the terms before you commit. In general, supplementary plans help pay for things like:

  • Specialist visits and diagnostic exams
  • Private hospital stays and surgery
  • Dental care and some vision care
  • Maternity-related services
  • Prevention checks and screening

Some plans extend to the employee’s family. Some do not. Coverage limits, exclusions and waiting periods differ, so compare them line by line.

Where collective agreements come in

Italian employment is shaped by national collective agreements, known as CCNL. Many of them provide for sector health funds or other welfare benefits for employees. Which agreement applies to your hire depends on the sector and the role.

This matters for two reasons. First, a benefit may be expected or required under the agreement, so you cannot treat it as optional. Second, if the agreement already provides a baseline, a company plan can top it up instead of duplicating it.

Check the applicable agreement before you design a benefits package. Do this before the offer, not after.

Why it matters for hiring and retention

Italian candidates know the system. They know what the waits can be like. A solid supplementary plan reads as a real signal that you take care of your people.

For foreign companies entering Italy, this is a cheap way to stand out. You may not match local brand names on salary or prestige. You can match them on benefits that people use.

A plan also helps with relocation. Someone moving to Italy from abroad is often unsure how to find care. A plan with clear access and English support removes a worry at the moment they need it most.

How to approach it

Keep the process simple.

  1. Identify the collective agreement that applies to the role.
  2. Find out what health benefits it already provides.
  3. Decide what you want to add on top, and for whom.
  4. Compare insurers on coverage, exclusions, provider networks and claims process.
  5. Explain the benefit clearly in the offer and in onboarding.

Finance teams should also check how the benefit is treated for payroll, since tax and contribution treatment of welfare benefits has its own rules. Get that confirmed locally before you roll out the plan.

The bottom line

Public care in Italy is real and valuable. It is not always fast, and it is not the same everywhere. Supplementary health insurance closes part of that gap. It gives employees speed, choice and peace of mind, and it gives you a stronger hiring offer.

The cost depends on the plan, the collective agreement and the size of your team. Before you decide, see what it adds to your total cost of employment. Try the cost calculator.
